Referrals to ACDP



Who is eligible?

Adults 18 years of age or older who live with:

  • ​aphasia
  • apraxia of speech
  • dysarthria or
  • cognitive communication disabilities due to a stroke, brain injury or brain illness.

Who can refer you?

  • Health care professionals or community agencies
  • Family members, friends, or supporters
  • Self-referral​​

Referral Process

1. Appy 

2. In home assessment
    A Speech Language Pathologist will:

  • visit the client's home
  • conduct a communication assessment
  • determine elegibility.

Home Assessment

3. Visit a Program
Vist a ACDP ProgramClient and family are invited to participate in one of our aphasia programs, after the in home assessment. 




4. Following the visit
mutual decision is made whether the program meets client needs and staff resources.
