Application Forms
The following application forms must be printed and filled in, preferably using CAPITAL letters and a black pen.
New applicants are required to complete one of the applications below. Once received, our lead conductor will contact you to arrange an initial consultation.
A Conductive Education® initial consultation takes place at our office in Toronto or Mississauga and usually lasts one hour. Consults cost $50, which will be reimbursed should you opt to register in a Conductive Education® session.
Admission and placement will be based on the consultation and assessment by the conductor. The conductor will also provide a written report which includes information on the applicant's current development and a clear summary of any action to be taken as a result of the consultation process.
