After you have reviewed the eligibility criteria, you can submit your completed and signed application with supporting documents to the Home and Vehicle Modification Program. Incomplete applications packages will not be processed for funding. One attempt will be made to secure missing information.
The program will not approve reimbursement of any cost for modifications, adaptations, and devices prior to authorization from the program.
Application Packages must be complete; only one follow-up attempt will be made to help you with the missing information.

information package for the specific funding category you are applying for.
Required Documents
1. Signed Application Form
2. Proof of Income
- Notice of Assessment from the most recent tax year.
- If applying for a stairlift, modular ramp, vertical platform lift, tub cut-out, or ceiling track lift and you own your home, you must provide a Notice of Assessment for each homeowner listed on the property tax bill.
- Blackout your SIN #.
3. Disability Verification
Choose one of the following:
- Occupational Therapist (OT) Assessment
- MODCs Verification of Disability Form (see below)
4. Price Quote
- Must be from a registered medical vendor/supplier or contractor.
- Include a detailed breakdown of costs.
5. Proof of Home Ownership or Rental
- Homeowners: Provide a current year property tax bill.
- Renters: Provide a written letter of approval from your landlord if applying for a stairlift, modular ramp, tub cut-out, automatic door opener or ceiling track lift.
- If you are a condo owner, a letter from the condo board approving your project is required. Please refer to Section 98 of the Condominium Act. Any fees associated with condo board requirements are not covered by March of Dimes Canada.
6. Additional Requirements for Specific Equipment
For custom/curved stairlifts, modular ramp, vertical platform lift, or ceiling track lift, include:
- Pre-installation photos
- Drawings
- Measurements
Note: These are not required for Straight Stairlifts.
7. Seasonal Restrictions
Applications for outdoor lifts and ramps (with concrete work) are only accepted from April 1 to October 31.
Occupational Therapist (OT)
To find an Occupational Therapist (OT), ask your family doctor or contact Ontario Health at Home at 310-2222 (no area code required) who can recommend publicly funded OT services in your community. You can also find more information online about Finding an OT at: https://otontario.ca/find-an-ot/.
Verification of Disability
Please download, print and fill out the following Verification of Disability Form, available in regular and large print, as well as in French (PAHV – Programme d’adaptation des habitations et des véhicules).
